What should I bring during my first apportionment?

Please bring the following: Insurance Card, a photo ID, and Prescription/referral for physical therapy (If needed)

Additionally, please arrive 10 minutes early to complete paperwork, so you can maximize your time in your evaluation.

How long are therapy appointments?

The initial evaluation is usually 60 to 80 minutes and each session after that will take approximately 30 to 60 minutes.

How much will I pay for therapy (With Insurance)?

Insurance benefits are normally verified by the time of 2nd visit, We will inform you of any out of pocket costs then.

How much will I pay for therapy (With No Insurance)?
